2010-2011 PFA APPLICATION PROCESS
STEP 1: "INTENT TO APPLY" SCREENING BY PHONE
STEP 2: PRE-PFA SITE VISIT & PROVIDER APPLICATION
- After screening, First 5 San Francisco staff will conduct a Pre-PFA Site Visit to review with interested providers the PFA Baseline Criteria, Program Assurances, Application, and answer questions regarding the process.
- During the Pre-PFA Site Visit, First 5 San Francisco will issue the PFA Provider Application documents. Providers that are ready to proceed in the Application process may complete documents and submit them by the due date (see Step 3, below).
STEP 3: APPLICATION REVIEW
- To ensure full consideration for Preschool For All participation beginning Fall 2010, submit your PFA Provider Application to First 5 San Francisco by 5:00 pm by Friday, January 29, 2010.
- Applications received after 1/29/2010 will be considered only after on-time Applications have been processed, and depending on funding availability.
- Submission of the PFA Provider Application does not guarantee funding.
- After reviewing Applications, First 5 San Francisco will notify programs that meet the PFA Baseline Criteria, and have submitted all required documents, to schedule an Application Site Visit with a review panel.
STEP 3-A: SUPPLEMENTAL APPLICATION
For all Applicants without a current, qualifying -ERS Assessment
- Providers without a valid –ERS Assessment(s) that meets the minimum requirements as stipulated in the PFA Baseline Criteria MUST complete a PFA Provider Application Supplement in addition to the standard PFA Provider Application documents. The Application Supplement is shared with instructions during the Pre-PFA Site Visit.
- If –ERS Assessment funding is available, qualifying providers may then be able to schedule an –ERS Assessment date with the Gateway To Quality Project.
- In order to proceed in the PFA Provider Application process, all providers must have a current qualifying –ERS Assessment from the Gateway To Quality Project. Sites that require new or re-assessment will have their PFA Provider Application held as “pending” the receipt of all documentation from the completed assessment activity. This will delay the PFA Provider Application process so the site visit and notification dates described below may not apply.
STEP 4: APPLICATION SITE VISITS
- First 5 San Francisco will conduct 2010/2011 Application Site Visits from February through March 2010. Application Site Visit details will be provided when programs are notified (refer to Step 2 – Application Review).
STEP 5: NOTIFICATION OF APPROVED SITES
- Sites approved to participate in 2010/2011 will be notified in April, 2010.
STEP 6: FUNDING AGREEMENTS
- All PFA Providers must sign a Memoranda of Understanding (MOUs) with First 5 San Francisco, which stipulate the various operational, programmatic, monitoring, and reporting requirements of the PFA program.
- Funding will be disbursed via the PFA Fiscal Services Agency. Each agency’s Funding Agreement is developed after an MOU is completed.
- Funding Agreements are renewable annually upon re-certification.
APPLICATION PROCESS AND CRITERIA
Remember, the 2010/2011 PFA Provider Application documents are only available from First 5 San Francisco staff during a pre-PFA Site Visit (see “Step 2”, above).
APPLICATION DUE DATE
Preschool For All Provider Applications must be complete and submitted with all required documentation (including Application Supplements, if applicable) by 5:00 p.m. on Friday, January 29th, 2010, to First 5 San Francisco, 1390 Market Street, Suite 318, San Francisco, CA 94102.
Applications must be submitted in hard copy, with original signature on page 14. Applications may be submitted by mail or in-person (no faxes or emails, please). First 5 San Francisco will send an acknowledgement when your Provider Application is received.
